The Multidisciplinary Model

Yolo Adult Day Health Center uses a model that combines several specialties in order to provide services and expertise in a wide range of areas to our participants, whose needs are often diverse:

All staff, each focused on one or many of the above services, are considered members of the multidisciplinary team (MDT). The MDT is considered the heart and soul of our approach to assistance. As a team we place emphasis on empowering people to achieve maximum independence which promotes a positive self-image and preserves dignity.
